TRYOUT LIABILITY WAIVER & PHOTO/VIDEO RELEASE
MASL2 Tryouts — Let’s Play ISA — November 8, 2025 (Check-in 6:30 PM)
Assumption of Risk & Acknowledgement
I understand that participation in soccer tryouts and related activities (“Activities”) involves physical activity and inherent risks, including risk of serious injury or death, and that these risks cannot be eliminated entirely. I certify that I am physically able to participate, and I voluntarily assume all risks associated with participation in the Activities.
Release of Liability, Waiver, and Covenant Not to Sue
In consideration of being permitted to participate in the Activities, I, on behalf of myself, my heirs, personal representatives, and assigns, hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Certified Lions FC, its owners, directors, officers, employees, volunteers, coaches, agents, representatives, sponsors, and the owners/operators of Let’s Play ISA (collectively, the “Released Parties”) for any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in the Activities, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.
Medical Authorization
I authorize Certified Lions FC and its representatives to obtain medical care for me or my child in case of emergency and agree to be financially responsible for any treatment provided.
Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, losses, or expenses (including attorneys’ fees) arising out of my participation in the Activities or my breach of this Agreement.
Photo/Video/Audio Release & License
I grant Certified Lions FC, its assigns, licensees, and successors the irrevocable, worldwide, perpetual right to photograph, videotape, and/or record my image, voice, and likeness and to use, reproduce, distribute, display, and create derivative works of such recordings and images for any lawful purpose including promotion, advertising, publicity, social media, and commercial materials, in any medium now known or hereafter developed, without further notice or compensation to me. I waive any right to inspect or approve the finished product(s) incorporating my likeness and release the Released Parties from any claims related to such use, including claims for invasion of privacy, defamation, or infringement of moral rights or publicity rights.
Ownership & Royalties
I acknowledge that all such photos, videos or audio recordings shall be the exclusive property of Certified Lions FC and that I will not be entitled to any royalties, residuals, or other compensation for their use.
Minors (if Participant is under 18)
If the participant is under 18 years of age, a parent or legal guardian must sign below. By signing, the parent/guardian affirms they have the legal right to consent on behalf of the minor, acknowledge and accept the terms above, and agree to indemnify and hold harmless the Released Parties for claims brought by or on behalf of the minor.
Severability
If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
Acknowledgement of Understanding
I HAVE READ THIS AGREEMENT, UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
